Kaiser Permanente works with local city and county governments and nonprofit organizations (our Community Partners) to provide the Kaiser Permanente Bridge plan.
To apply for the Bridge Plan, please mail your completed application and some basic information to one of Kaiser Permanente's Community Partners in Northern Virginia, Washington, D.C., and Maryland. Please call or email your Community Partner with any questions about the application. Use this list to find a Community Partner near you.
